 Try (probably there is a better way to do it)

 cd ext/tcltklib
 ruby extconf.rb --with-tk-include=/usr/X11R6/include \
                 --with-tk-lib=/directory/where/is/libtk
 make
 make install

 or  if tk.h is in /usr/X11R6/include and libtk in /usr/X11R6/lib

 ruby extconf.rb --with-tk-dir=/usr/X11R6


 Same with --with-tcl-include and --with-tcl-lib if it don't find tcl.h or
 libtcl (you have also --with-X11-include, --with-X11-lib and
 --with-X11-dir)
